The Eliza Bryant Village Senior Outreach and Adult Day Services program provides nurturing care for older adults in a safe environment while offering respite to caregivers Eliza Bryant Villages’ flexible Adult Day Services program offers a variety of services that best suit the needs of your aging loved one.  From 7:30 a.m. to 4:30 p.m., Monday through Friday, our professional and compassionate staff provide care and support while your loved one enjoys engaging activities and amenities in a welcoming, therapeutic and comfortable setting.

While in our care, your aging loved one will benefit from:

Adult Day Care Program

Unique to the Village is our unique enhanced medical model program that tailors services to older adults with cognitive and physical impairments.  We have a full-time nurse and a full-time social worker who assist with monitoring of chronic conditions through blood pressure and glucose checks, and administration of medication as prescribed by a physician.  We also provide therapeutic groups and services that help the mind, body, and spirit for a better quality of life.  All therapeutic groups are facilitated by certified and licensed professionals.

  • Music Therapy – improves mood, cognition, memory, and social interaction while inspiring movement
  • Art Therapy – reduces stress and anxiety, improves well-being, hand dexterity, and hand/eye coordination
  • Massage Therapy improves circulation, flexibility and better sleep while strengthening immunity and reducing pain and inflammation
  • Trauma Therapy – promotes healing and reinforces a sense of safety, self-worth, and empowerment
  • Physical Therapy – reduces pain, improves mobility, balance and prevents falls and helps manage diabetes and vascular conditions
  • Speech Therapy – strengthens knowledge and memory processes and provides strategies for mealtime and intake of food and drink
  • Occupational Therapy – helps maintain cognition and functionality and supports meaningful activities of daily living

Eliza Bryant Village’s Adult Day Program offers specially tailored programs and services to assist seniors with a wide variety of activities that help maintain independence, promote feelings of self-worth and help seniors stay in their homes longer. As we listened to the needs of our participants and the concerns of their caretakers, we learned we needed to improve our services for emotional and mental health. With funding from the United Black Fund, we created a Relaxation Room that helps participants find a safe space to take time to themselves to think, relax, and gather themselves before returning to activities with their peers. We also added a new service – massage therapy – that helps reduce problems associated with chronic stress such as depression, anxiety and hypertension, as well as to heal physical aches and pains of aging.

Here are a few things to keep in mind:

  • We accept a variety of referral sources including the Western Reserve Area on Aging, VA Hospital, County Board, Hospitals, Nursing Homes, other Adult Day programs and family members. 
  • We accept a variety of payment forms including the Community Social Service Program, Cuyahoga County Board of Development Disabilities, Passport Waiver Program, Private Insurance,  Title III (upon request) and the Veterans Administration.
  • We can provide transportation! Click here to print and sign our transportation waiver.
  • We are here to help you! We want this process to be as easy as possible.

Complete and bring this medical form with you to your appointment. Since we need evidence of a negative TB prior to enrollment, we offer TB shots for a modest fee of $20. Let us know if you need one!

Here is a list of additional information that should be brought to your appointment:

  • Personal information (address, phone number, social security number)
  • Name and telephone number of Primary Care Physician
  • Driver’s license or State ID or Picture ID
  • Social Security card or Birth Certificate
  • Medicaid, Medicare or other insurance cards
  • Do you have a Case Manager? If no, skip this step! If you do, please bring their contact information and an authorization form.

Let’s talk about how you’d like to pay. We accept Community Social Service Program, Cuyahoga County Board of Development Disabilities, Passport Waiver Program, Private Insurance, Private Pay, Title III and the Veterans Administration.

Do you need transportation? We can help with that! Tell us where you live, what time you’d like to arrive and depart, and what days you’d like to participate.

Meet the team! Join us for a short and sweet orientation. We offer these brief meetings on the first Friday of each month from 12:30 p.m. to 1:30 p.m. and we provide lunch. We ask that you please attend orientation within 60 to 90 days of enrollment in our program.


Our Medical-Model – What It Means & How it Benefits You!

Mobile Dental Clinic

Eliza Bryant Village with Case Western Reserve University School of Dental Medicine’s Lifelong Smiles program offers a monthly Mobile Dental Clinic.

Lifelong Smiles works to deliver comprehensive dental care to the elderly population in adult day care centers, assisted living facilities, and nursing home facilities. The Mobile Dental Clinic offers:

  • Oral exams
  • X-rays
  • Cleanings
  • Fillings
  • Extractions
  • Dentures and partials

Services are offered on the last Tuesday of each month. Medicaid is the accepted method of payment and appointments are required.

Mobile Eye Care Clinic

With Family Eyecare Clinic, Eliza Bryant Village offers a monthly Mobile Eye Care Clinic for program participants.

The Mobile Eye Care Clinic provides vision care using state-of-the-art equipment to examine eyes, prescribe glasses and treat eye conditions including glaucoma and diabetic eye disease.

Podiatry Clinic

The Eliza Bryant Village Adult Day Program offers a Podiatry Clinic on the last Wednesday of each month in our center.

Dr. Adam Warren, DPM, provides basic foot care, such as treatment of bunions, calluses, corns, nail disorders and spurs.

Appointments are required. Call Ms. Mitchell, Senior Outreach Nurse, to make an appointment at 216-361-6141 ext. 885.


Caregiver Support Groups

Caregiver Support Groups, held on the fourth Thursday of every month, provides a forum or hosts a guest speaker about a specific topic where caregivers can exchange ideas, relieve stress and meet new friends who understand your situation.

The Eliza Bryant Village Caregiver Support Group provides a safe space for caregivers to connect with new friends and learn tools and resources for coping with stress, anxiety and caring for aging adults. Our confidential Caregiver Support Group meets on the fourth Thursday of every month at 6:00 p.m. These meetings are open to the public and we invite you to join us to connect with professional guest speakers, staff and view our center.

Home Care Program

Eliza Bryant Village Home Care helps our neighbors Age In Place and stay in their home longer. Our compassionate Personal Care Assistant team helps with activities for daily living and hygiene care and offer compassionate and reliable non-medical services for seniors in the comfort of their own home.  Our Home Care services include:

  • Personal care
  • Dressing
  • Meal preparation
  • Medication reminders
  • Light housework and laundry
  • Errands
  • Medication pickup
  • Transportation to medical appointments if applicable

Home Care Enrollment Process

  1. Connect with a member of the Eliza Bryant Village Home Care team
  2. We will confirm your insurance provider and the level of care needed
  3. Eliza Bryant Villages’ Senior Outreach Nurse will conduct a home assessment and create an Individualized Plan of Care
  4. Let’s sort through the details:
    1. When do you want your service to start?
    2. What’s the best contact number for you?
    3. How often do you want assistance?
  5. Our Home Care services begin!
  6. We’ll provide bi-monthly visits and assessments to make sure you are satisfied with your services.

Community Transportation

Eliza Bryant Villages’ Community Transportation Program provides older adults in the community safe and reliable transportation to help with neighborhood errands like picking up medication, medical appointment transportation or going to visit a loved one. Round trip rides are available for $5 per person, per trip. Free rides are available for those who qualify.  Riders must live within a three-mile radius of Eliza Bryant Village to be eligible for service.

Reservations must be made at least one day in advance. Transportation is confirmed on a first-come, first served basis.  For that reason, it is advised that trips be planned and confirmed with as much notice as possible to increase the likelihood that transportation is available for your request.

Transportation is available Monday through Friday between the hours of 10:30 a.m. and 1:00 p.m.  Vans are needed at the Village at 2:00 p.m. to transport participants back home.
