Helping Your Loved One Stay at Home Longer While Offering Respite for Caregivers

We offer specially tailored programs and services to assist seniors with a wide variety of activities that help maintain  independence, promote feelings of self-worth and help your aging loved one stay in their home longer.

Our unique enhanced medical model program tailors services to seniors with cognitive and physical impairments through daily enrichment activities. This medical model program includes traditional services such as social service, activities, and crafts and also includes critical medical skilled services from nurses, therapists, social workers, trained dementia staff and geriatric physicians.

Our programs and services offered Monday through Friday from 8:00 a.m. to 5:00 p. m. include:

  • Nutritious meals (breakfast, lunch and snack)
  • Fitness and wellness programs
  • Caregiver support groups
  • Music and art therapy
  • Cultural activities and field trips
  • Wheelchair accessible shower and laundry services
  • MMonitoring of chronic conditions through blood pressure and glucose checks and administration of medicine as prescribed by a physician
  • Dental, optometry and podiatry services
  • Physical, speech and occupational therapy
  • Safe and reliable curb to curb transportation

Caregiver Support Groups

Caregiver Support Groups, held on the last Thursday of every month, provides a forum or hosts a guest speaker about a specific topic where caregivers can exchange ideas, relieve stress and meet new friends who understand your situation.

The next meeting will be held Thursday, March 22, 2018 at 6:00 p.m.

Home Care Program

Home Care services help our aging neighbors stay in their home longer by providing help with activities for daily living and hygiene care, assistance with minor household chores such as cleaning and laundry, help with errands such as picking up medication and transportation to medical appointments.

Community Transportation

Our On-Demand Transit Program provides seniors in the community safe and reliable transportation to help with neighborhood errands like picking up medication, medical appointment transportation or going to visit a loved one. For just $5 (round trip), rides are available Monday through Friday between 10:00 a.m. and 2:00 p.m.  Reservations must be made one day in advance.
